Is this a confirmed appointment?

No. This form is a request. A coordinator will contact you to confirm the date, time, clinician, and visit type before it is booked on the schedule.

How soon will someone contact me?

Most requests are reviewed the same business day. After hours and weekend requests are handled the next morning the clinic is open.

Can I request a telehealth visit?

Yes. Choose Telehealth under Visit type. We will confirm whether your concern is appropriate for a video visit.

What should I bring?

Photo ID, insurance card if you have one, a list of medications and allergies, and any recent labs or records related to your visit.

Do you accept new patients?

Yes. Select New patient as the visit type so we can send intake forms before you arrive.
